Bilingual LPN/LVN – Spanish Speaking

Role Overview

• Conduct telehealth visits in Spanish and English
• Review patient-reported data and RPM alerts
• Document care in compliance with CMS and internal standards
• Collaborate with interdisciplinary care teams
• Support post-discharge and follow-up coordination

Requirements

• Active Nevada LPN or LVN license
• Professional fluency in Spanish and English
• Minimum 2 years clinical or telehealth experience
• Strong documentation, clinical judgment, and communication skills
• Experience with CCM or RPM preferred
• Ability to work independently in a HIPAA-compliant environment
