Assist in managing the health of your panel of patients and proactively track whether your patients are up to date on preventative measures
Assist in the facilitation of referrals, prior authorizations, and lab order fulfillment
Proactively follow-up with patients regarding logging vitals (BP, BS, weight), symptom assessments, and scheduling needs
Aid patients in the fulfillment, initiation and follow-up for prescriptions
Help patients and family members understand their chronic conditions and how to prevent further disease progression
Ensure that care provided is collaborative and act to engage specialists as needed
Assist clinicians as needed
Other duties as assigned

Excellent time management skills and ability to demonstrate high attention to detail
Electronic health record (EHR) experience; Athena and Salesforce experience preferred
Medical Assistant certification preferred
Outstanding communication skills with exceptional listening ability
Ability to be flexible and available as needed throughout the workday
Exceptional interpersonal skills with the ability to develop trusting, collaborative relationships
Desire to help patients live a better, healthier life
