Role Overview
- Provide support to patients and health care providers
- Assist patients and caregivers in conducting assessments
- Act as a liaison between patients, families and health care providers
- Offer continuous support on disease states or product-related questions
- Refer the patient to healthcare providers for clinical questions
- Document patient information and interactions
- Conduct ongoing file review and updates
- Follow case-management protocols
- Collaborate with reimbursement specialists and physicians
- Maintain clear communication with the manager and team
Requirements
- A Diploma in health care or a related field
- Licensed Nurse (RN or RPN/LPN) with active registration
- Minimum of two (2) to five (5) years in healthcare, pharmaceutical, insurance or related fields
- Fluency in French and/or English may be required
