Role Overview
- Provides case management/therapy for patients and families to enhance their psychosocial functioning
- Performs duties and responsibilities in a manner consistent with mission, values and mercy service standards
- Position will be serving collaborative care
Requirements
- Master's degree required in Social Work, Counseling or another human-service related field
- Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) required
- Minimum 2-3 years Behavioral Health experience preferred
- Co-worker must be able to communicate orally, in writing and demonstrate proficient computer skills
- Co-worker must be able to listen and to understand and explain complex situations
