About this role
Role Overview
Build trusted relationships with members through in-person visits (approximately 70% travel)
Conduct comprehensive assessments using clinical judgment and critical thinking to understand medical, behavioral, and social needs
Develop and implement individualized care plans that promote health, independence, and quality of life
Collaborate closely with providers, community resources, and interdisciplinary teams to ensure coordinated, high-quality care
Connect members to services, supports, and resources that align with their goals
Ensure care delivery meets regulatory requirements and Aetna quality standards
Advocate for medical appropriateness while supporting positive member engagement and outcomes
Requirements
2+ years of experience in behavioral health, social services, or a related field aligned with program focus
Residency in one of the following Virginia areas: Alexandria, Arlington, Vienna, McLean, Woodbridge, and/or Springfield
Reliable transportation and the ability to travel up to 70% of the time (mileage reimbursed per company policy)
