Conduct outbound patient outreach calls in both English and Spanish.
Perform health risk assessments and support CCM and RPM activities.
Document all patient interactions accurately within Electronic Health Record (EHR) systems.
Educate patients and families on chronic disease management and treatment adherence.
Identify gaps in care and escalate concerns appropriately to providers or internal teams.
Coordinate care plans and maintain consistent patient follow-up schedules.
Connect patients with community resources and healthcare support services.
Maintain HIPAA compliance and confidentiality standards at all times.
Collaborate with providers and care teams to improve patient engagement and outcomes.

Active, unencumbered LPN/LVN license (California or compact state accepted).
Residency within the MST or PST time zone.
Professional fluency in both English and Spanish.
Current Basic Life Support (BLS) certification.
Strong proficiency with EHR systems and Google Suite.
Dedicated, HIPAA-compliant home office with reliable high-speed internet and dual-monitor setup.
Excellent clinical reasoning, communication, and patient engagement skills.

Nice to Have

Previous experience in care coordination, CCM, TCM, or remote patient monitoring programs.
Experience educating patients and families on chronic disease management.
