Provide a comprehensive approach to case management inclusive of all aspects of the patient's stay
Perform and execute discharge planning
Develop sound and reliable discharge plans incorporating risk for readmission assessment
Include all stakeholders in the discharge plan
Participate in multidisciplinary rounding process
Work collaboratively with all caregivers in the continuum of care
Organize and lead multidisciplinary care conferences
Identify barriers to transportation and assist with getting patient transportation to appointments
Provide ongoing recovery support service
Document all discharge planning components accurately in EMR

Bachelor's Degree: Social Work; Nursing; Related Field
Basic Life Support (BLS) within 30 days
American Heart Association (AHA)
Driver's License
Illinois Secretary of State (ILSOS)
Must have control over personal use of all mood-altering chemicals and behaviors
If recovering from an addiction, must currently have a minimum of 2 years of uninterrupted recovery
Advanced reading skills
Advanced writing skills
Advanced oral skills
Ability to respond appropriately to customer/co-worker interaction
Maintain confidential information
Ability to communicate only the facts to recipients or to decline to reveal information
Ability to project a professional, friendly, helpful demeanor
Basic computer knowledge: Uses word processing, spreadsheet, e-mail application, and web browser
Comfortable within a Windows OS and learning new applications.
