RN, Case Manager – Home Health

Role Overview

• Provides professional nursing care to assigned clients
• Ensures quality and safe delivery of home health care services
• Conducts client assessments and completes visit documentation
• Develops and amends client's Plan of Care
• Communicates changes in client's condition to physician
• Reports and completes appropriate documentation regarding client occurrences

Requirements

• IL RN License
• BLS within 30 days
• Drivers License
• Proof of Auto Insurance
• 1yr of acute care nursing experience
