About this role
Role Overview
- Provide integrated support to patients and physicians
- Develop personalized relationships to improve medication adherence
- Review and interpret clinician's patient care notes
- Develop and refine disease and drug specific clinical protocol
- Monitor ongoing patient adherence across disease categories
- Evaluate EMR for medication adherence and optimize outcomes
- Recommend alternative therapeutic drug regimens
- Monitor and evaluate therapies for side effects
- Conduct medication therapy management (MTM)
- Implement clinical interventions and improvement initiatives
- Maintain compliance with local, state and federal drug laws
- Supervise support personnel compliant with HIPPA rules
- Participate in education programs and train pharmacy staff
Requirements
- Applicant must hold a Doctor of Pharmacy (PharmD) or equivalent advanced degree
- 1+ years' experience strongly preferred but not required
- Knowledge using EPIC/QS1/WILLOW software a plus
- Deep knowledge of specialty medication pharmacology
- Strong verbal and written communication skills
- Must be computer literate with proficiency in Microsoft Word, Excel, and PowerPoint.
- Ability to identify barriers to medication adherence
- Motivating and coaching patients to achieve better outcomes
