Provide integrated support to patients and physicians
Develop personalized relationships with patients and prescribers
Review and interpret clinician's patient care notes for supportive clinical data
Develop disease and drug specific clinical protocols
Monitor ongoing patient adherence across all sites
Evaluate EMR to review interactions, medication selection, labs, allergies, and communication
Recommend alternative therapeutic drug regimens
Monitor therapies for side effects and recommend changes
Manage and document medication therapy management
Oversee tech's daily activities
Identify and resolve insurance related issues
Maintain HIPPA compliance
Train pharmacy staff, students, interns, and healthcare professionals
Assist with research projects and peer reviewing articles

Required Licensure/Certification: Applicant must hold a Doctor of Pharmacy (PharmD) or equivalent advanced degree and be a licensed Pharmacist
1+ years' experience is strongly preferred but not required
Knowledge using EPIC/QS1/WILLOW software a plus
