Role Overview

• Provide integrated support to patients and physicians
• Review and interpret clinician's patient care notes
• Develop and refine disease and drug-specific clinical protocols
• Monitor ongoing patient adherence across all sites
• Recommend alternative therapeutic drug regimens
• Medication therapy management
• Direct communication with health professionals
• Supervise support personnel compliant with HIPAA rules

Requirements

• Doctor of Pharmacy (PharmD) or equivalent advanced degree
• Licensed Pharmacist
• 1+ years of experience preferred
• Knowledge of specialty medication pharmacology
• Proficiency in Microsoft Word, Excel, PowerPoint
• Knowledge using EPIC/QS1/WILLOW software is a plus
