Physical Therapist, Team Resource

Role Overview

• Coordinate patient care for assigned caseload
• Accept accountability for patient satisfaction scores
• Develop and implement patient specific plan of care
• Administer medications and treatments as prescribed by the physician
• Communicate with team disciplines and community providers to coordinate care

Requirements

• Physical Therapist with current licensure to practice therapy in the state of Massachusetts
• Minimum of 1-2 years physical therapy experience
• OASIS experience preferred
• Must be a licensed driver with an automobile that is insured
• Self-directed and able to work with minimal supervision
