Physical Therapist, Team Resource

Role Overview

• Coordinate patient care for assigned caseload
• Accept accountability for patient satisfaction scores
• Develop a patient specific plan of care
• Initiate appropriate preventive and rehabilitative therapy procedures
• Communicate with the physician regarding the patient’s needs
• Attend regular case conferences and team meetings

Requirements

• Minimum of 1-2 years physical therapy experience, at least one of which is in the area of public health or home care
• Current licensure to practice therapy in the state of Massachusetts
• Active American Health Association BLS
• Graduate of an accredited program in Physical Therapy; DPT preferred
• Must be a licensed driver with an automobile that is insured
• Self-directed and able to work with minimal supervision
