Ability to provide comprehensive medication management including initial assessments, toxicity evaluations/symptom management, and reassessments.
Requirements
PHARMD-Graduate of an accredited pharmacy school
Current registration or Registered Pharmacist within 120 days
Experience as a specialty pharmacist in a hospital setting
Board Certification (ex: BCACP/BCPS/BCOP/CSP)
EPIC and Beacon experience
ASHP-Post Graduate Year 1 ASHP General Pharmacy Residency
Knowledge of medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations and laws and regulations.
Knowledge and understanding of different Pharmacy practice settings, including narcotic delivery and procedures.
General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook)
