Ensure continuity and quality of services by managing temporary and complex cases
Provide person-centered case management services to individuals with disabilities, mental health conditions, or terminal illness across multiple counties
Act as a leader within the team, supporting new hires and stabilizing cases
Advocate for individuals to achieve personal goals through community resources
Document all work; exceed minimum billing standards
Requirements
Bachelor's Degree in social work, nursing, psychology, sociology or a closely related field from an accredited four-year college or university; OR
A bachelor’s degree from an accredited four-year college or university in a field other than social work, nursing, psychology, sociology or a closed related field AND one year of experience in the delivery of social services to the target population as a supervised social worker in a public or private social services agency.
If you hold a Social Work degree, we expect you to be licensed by the Minnesota Board of Social Work, as required.
