Meeting potential and current clients in their homes to listen to their stories, determine their needs, and establish a plan of care during an initial assessment.
Managing the coordination of care and maintaining compliance with policies and procedures and all state home care rules and regulations.
Provides home visits on an as needed basis or every 90 days to monitor any changes in client condition.
Oversee the Caregivers providing care to our wonderful clients.
Assist in New Hire Orientation and Training of new Caregivers, as well as perform skills assessment on New Hires.
Observe Caregivers in the home and assist in employee reviews.
Requirements
Two years of Registered Nurse experience preferred but not required in home care, home health or related field
Reliable transportation with current insurance and a valid driver’s license.
Must possess excellent written and verbal communication skills and be able to interact effectively with a diverse audience including clients, families, care providers and other professionals.
Must possess impeccable follow up and attention to detail and be able to exercise initiative, problem solve, apply sound judgment.
Able to supervise, assess and evaluate care providers and advocate on behalf of clients and families.
