Role Overview
Do you have the right skills in delivering intensive support to individuals to enable them to provide the very best environments to Adults in the community to enable them to thrive?
You will be responsible for ensuring the safety and well-being of individuals within our community.
Conducting thorough assessments, collaborating with multidisciplinary teams, and developing comprehensive intervention plans to support at-risk individuals.
You will demonstrate exceptional communication skills, case management expertise, and the ability to navigate complex emotional scenarios with empathy and professionalism.
Excellent recording and report writing skills and IT are required for this role.
You need to be a team player, a good communicator, and a skilled and self-motivated professional, with the ability to work flexibly, as well as managing a caseload.
Requirements
Ability to carry out assessments, plan and deliver needs-based packages of support, balancing risk and protective factors.
Ability to build respectful, professional relationships with adults, families and carers, and to communicate effectively with people who may be experiencing distress, uncertainty or change.
Ability to gather information, listen carefully and contribute to assessments and reviews in a thoughtful and organised way.
Ability to work within clear guidance and delegated responsibilities, recognising when to seek advice, supervision or support.
Ability to identify concerns, risks or safeguarding issues and take appropriate action by following agreed procedures.
As this post meets the requirements of the Immigration Act 2016 (part 7), the ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post.
