Role Overview
- Be a vital member of each patient’s core care team.
- Help patients understand their rights and responsibilities.
- Guide patients in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.
- Build meaningful and long-term relationships with patients and their families.
- Support and advocate for patients in an intimate outpatient setting.
Requirements
- Meets all state required regulations to practice in the dialysis setting.
- Licensed Master's degree in Social Work (MSW) required
- Demonstrated knowledge of government and private insurance programs
- Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
