Role Overview
- Support and advocate for patients undergoing dialysis
- Help patients understand their rights and responsibilities
- Guide patients in managing physical, mental, emotional, and financial demands of End Stage Renal Disease
- Build meaningful and long-term relationships with patients and their families
- Collaborate as a vital member of each patient’s core care team
Requirements
- Meets all state required regulations to practice in the dialysis setting
- Master's degree in Social Work (MSW) required with a specialization in clinical practice
- Must have LMSW or RCSW
- Demonstrated knowledge of government and private insurance programs
- Basic computer skills in MS Word, Excel, PowerPoint and Outlook
- Functional proficiency with DaVita specific applications within 60 days
