Providing outreach to hospice families to determine their grief support needs.
Completing a bereavement assessment of the hospice family’s needs and developing an individualized plan of care based on the bereavement assessment.
Providing grief counseling to the bereaved on a time-limited basis as deemed helpful and appropriate.
Referring to the community those bereaved who need mental health or other community resources when needed.
Maintaining clinical records related to grief support services performed.
Promoting grief education and support to the hospice team on an individual or group basis.
Participating in the Interdisciplinary Team Meetings to determine grief support needs of hospice families.

Master’s degree in social work from an accredited graduate program.
Minimum of one year of experience as an LPC or LSW (preferred), healthcare or hospice experience preferred.
Bilingual proficiency in English and Spanish is strongly preferred.
Experience counseling children and teens is highly preferred.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and is in good working order.
