Role Overview
- join our team as a Care Coordinator on a full time ongoing basis
- work within the Emergency Department at Monash Medical Centre
- manage complexity in a fast paced Emergency Department
- provide support across Casey, Clayton, and Dandenong Emergency Departments
- lead discharge planning and care coordination
- liaise with external service providers to enhance patient care
- develop and apply transdisciplinary skills
- work collaboratively with a diverse team of Allied Health clinical experts
Requirements
- experienced Social Worker with a strong background in Emergency Department and/or acute hospital settings
- advanced clinical reasoning
- assessment and discharge planning skills
- experience providing clinical education, supervision and support to staff
- transdisciplinary clinical experience or eagerness to develop skills
- experience in leadership, quality improvement and/or service development highly regarded
- strong understanding of service systems such as Family Violence, Child Protection, My Aged Care, Addiction Medicine, NDIS, Mental Health, and recovery model
