About this role
Role Overview
Provide direct case management services to individuals who are needy or disenfranchised in Sullivan County.
Conduct initial intakes utilizing the Apricot Platform database system.
Actively participate in system-wide data sharing through integrated digital referral systems.
Prioritize intakes and ensure warm handoffs throughout the CCOSU system and with community partners.
Attend Parent-Child Program training and deliver services effectively in accordance with program guidelines.
Participate as a member of an interdisciplinary treatment team.
Maintain current, accurate, and complete documentation of all services provided to clients.
Answer telephones, provide information, and assist clients as needed.
Develop and maintain effective working relationships with service providers in Sullivan County.
Attend community and service provider meetings as required.
Participate in outreach activities during weekends and after-hours.
Maintain an updated community resource list/guide to use as a referral source for clients.
Provide crisis intervention services to homeless individuals and families experiencing housing emergencies.
Serve as a primary case manager for emergency relief funds.
Conduct thorough needs assessments and develop short-term service plans to address identified needs.
Conduct social care screenings of Medicaid members using standardized tools.
Log all client interactions thoroughly in required systems.
Maintain accurate and complete case files, documentation, records, and other required materials.
Process cases within a reasonable timeframe, preferably within 45 days.
Assist weekly with food and children’s pantry distributions.
Participate in additional outreach and special projects.
Attend required agency in-service trainings.
Travel to meet clients at other CCOSU offices as needed.
Provide coverage on other sites as needed.
Requirements
Associates degree with one-year experience in Human Services required.
Bachelor’s degree in social work or related field, highly preferred.
Must possess and maintain a clean NYS Driver’s License throughout employment and have reliable transportation.
Knowledge of Social Services available in Sullivan County preferred.
Bilingual (English/Spanish). Preferred but not required.
Computer literate with knowledge of Microsoft Word, Excel, and Outlook.
