Role Overview
- Provide nursing, case management/disease management services to clients and families
- Assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care
- Collaborate with the interdisciplinary healthcare team to ensure optimal patient outcomes
- Document assessments, interventions, and patient education in the electronic medical record
Requirements
- Current Georgia RN license from an accredited college
- BSN required or enrolled within 6 months of hire and BSN completed within three years from hire date
- BLS (Obtain AHA certification within 30 days of hire)
