Evaluates, plans, and implements specific treatment programs for individual patients according to the principles and practices of occupational therapy. Functions as an interdisciplinary team member providing patient assessment/re-assessment, direct patient care and individualized care planning/implementation.
- Performs evaluations/assessments on all patients; assessment and reassessment of patient is clearly and concisely performed following department guidelines and policies including but not limited to muscular strength, ROM, sensation, motor function, ambulation, balance, pain and social/environmental needs.
- Able to formulate an individualized plan of care with short term and long term goals, revises plan as indicated by patient’s response to treatment; documentation of treatment and progress meets current standards, policies and procedures. Goals have set time frames, are functional, measurable and specific.
- Patient/family/caregivers are involved in establishing treatment goals and plan of care with appropriate documentation.
- Treatment plan of care is modified based on ongoing reassessments, patient response to treatment rendered, change in medical status with documentation and procedures per department guidelines.
- Carries out established techniques for safe administration of treatment per SFH policies and departmental guidelines. Correctly administers treatment based on age of the patient and patient’s clinical condition.
- Performs duties as assigned.
- Required:
- Education: Bachelor's or master's degree in occupational therapy
- Licensure / Certification: State licensure
- Preferred:
- Experience: 2 years as a practicing OT in a healthcare setting
