RN Care Manager - Cardiovascular - Days

The Cardiovascular Case Manager is responsible for planning, coordinating, negotiating and initiating the efficient movement of the patient through the continuum of care, based on medical needs, functional ability and available resources. This position requires population related competencies. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires. The CV Case Manager responsibilities include, but are not limited to, the following:

• Assesses the patient continuously for medical necessity, appropriate level of care, financial situation and discharge barriers
• Responsible for completing physical assessments upon admission
• Initiates discharge planning and will collaboratively work with the interdisciplinary team to identify and coordinate needed services and resources
• Provides education to patient's family and staff as required
• Audits data abstraction for accuracy and provides concurrent feedback to physicians regarding quality measures The CV Case Manager reports to the Lead CV Case Manger. REQUIRED QUALIFICATIONS

LICENSE/CERTIFICATIONS:

• RN (Registered Nurse) Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state
• BLS (Basic Life Support) Issued by American Red Cross or American Heart Association within 30 days of hire

Must be able to communicate effectively in English (verbal/written).

PREFERRED QUALIFICATIONS

EXPERIENCE:

• 5 years clinical experience in an acute care environment OR 3 years experience with a Bachelors degree in Nursing
• Cardiology experience
