Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home.
Schedule: 8:30am-5:00pm
Position Summary:
The Intake Nurse Coordinator is responsible for developing and implementing a strategic plan to increase program enrollment for PACE. This position leads the initiatives and ensures the enrollment process is efficient and easily accessible for potential enrollees.
Position Qualifications:
▪ RN with current RN licensure in the state of Kentucky.
▪ Minimum two years nursing experience with geriatric population.
▪ Working knowledge of Medicare and Medicaid processes, payments, and requirements strongly preferred.
▪ Excellent communication skills, with an ability to adapt communication style to a wide variety of audiences and strategies.
▪ Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and multi-task with high level of quality and productivity.
▪ Strong analytical and problem-solving skills.
▪ Flexibility to travel throughout the PACE service area.
