Our Care Partners are supported by modern systems, streamlined processes and strong operational support, helping reduce administrative pressure so you can focus on meaningful client relationships and outcomes.

Due to continued growth, we are looking for an experienced Care Partner / Care Manager to join our team on a permanent full-time basis, based from our Carrum Downs office with hybrid flexibility. 

The Role 

You will support a portfolio of clients receiving Support at Home Packages, ensuring they receive personalised, high-quality care that enables them to remain independent at home. 

Key responsibilities include: 

• Conducting client assessments and developing tailored care plans 
• Coordinating services with healthcare professionals, support workers and families 
• Monitoring and reviewing care plans as client needs change 
• Supporting clients and families through their Support at Home journey 
• Maintaining accurate documentation and meeting compliance requirements 

About You 

We’re looking for someone who is compassionate, organised and client-focused, with a passion for delivering great outcomes in community aged care. 

You will have: 

• Experience as a Care Manager / Care Partner within Support at Home packages 
• Strong communication and relationship-building skills 
• Excellent organisation and time management 
• Understanding of the Support at Home or HCP program 
• Confidence using electronic care management systems
