Job Summary
Join our award-winning, 5-star hospital-based Skilled Nursing Unit We are seeking a dedicated and compassionate Registered Nurse to join our hospital-based Skilled Nursing Unit. In this role, you will bridge the gap between acute hospital care and home for patients requiring temporary rehabilitative support. You will provide high-level medical care to patients recovering from surgeries, complex illnesses, or chronic condition flare-ups (such as COPD or heart failure). As a key member of our interdisciplinary team, you will balance clinical expertise with leadership to promote patient independence and prevent hospital readmissions.
Key Responsibilities
- Comprehensive Assessment: Conduct thorough head-to-toe physical and mental health assessments upon admission and monitor for subtle changes in condition.
- Clinical Interventions: Administer oral and IV medications, manage central lines, and perform advanced wound care for surgical sites or pressure injuries.
- Care Coordination: Collaborate with physicians, physical therapists (PT), and occupational therapists (OT) to implement and update personalized care plans.
- Patient Advocacy & Education: Serve as a liaison between families and the medical team, educating patients on managing their recovery at home.
- Supervision: Oversee and delegate tasks to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs) to ensure a high standard of patient-centered care.
- Documentation: Maintain accurate, real-time records of vitals, treatments, and patient progress.
Shifts: Must be available 7p-7a; we are 24/7 365 days and our team works to care for our patients as needed whether week night or weekend night.
Requirements
Qualifications & Skills
- Licensure: Current and valid Registered Nurse (RN) license in the state of NC.
- Certification: Current Basic Life Support (BLS) and/or CPR certification.
- Experience: Previous experience in skilled nursing, med-surg, or acute care is preferred; new graduates with strong clinical rotations are encouraged to apply.
- Soft Skills: Strong critical thinking, emotional resilience, and the ability to communicate empathetically with stressed or vulnerable patients and families.
- Physical Stamina: Ability to stand for long periods and assist with patient transfers or repositioning (up to 30 lbs).
