JOB SUMMARY: This position will provide Prevention navigation services, including but not limited to HIV/STI
testing, pre- and post-test counseling, condom education and distribution, and referrals and/or linkage to prevention and essential services for high-risk HIV-negative persons and persons living with HIV. This position has a mandatory outreach requirement. Staff are required to participate in off-site community outreach activities, including evenings and weekends as needed, and must
provide HIV testing, pre-and post-test counseling, and linkage to rapid entry HIV medical care services during outreach events in accordance with PIHC and funder requirements. This position will provide rapid entry into HIV care services at PIHC, off-site, and through self-based testing in confidential, one-on-one sessions with clients while maintaining the pre-counseling protocol of
no more than 30 minutes per session. This position will fully support the PIHC PrEP program by providing PrEP and PEP education; linkage to same-day services; retention and re-engagement to PrEP care; and administrative support. Staff will utilize established Rapid Entry and Reengagement in Care protocols to link clients to prevention and essential services. This position will track linkage or re-engagement for up to 90 days to ensure clients access their first medical appointment; document all linkage and navigation services; and notify the local health department of all HIV-positive clients for Partner Services. This position reports directly to Prevention Program Manager I and supports targeted Prevention program objectives. This position requires fluency in Spanish and English to effectively serve PIHC’s diverse client population. The Bilingual Prevention Navigator will provide culturally and linguistically appropriate services, including interpretation, translation of prevention education materials, direct client navigation in Spanish to reduce barriers to HIV/ST testing, PrEP & PEP access, and rapid entry to care.

TRAVEL:
• Local travel between PIHC sites and to and from community agencies will be required.
• Occasional travel to events for training and promotion of salient services to AIDS Service
Organizations. Occasional evening and weekend work is required and working greater
than 40 hours per week may be required
