The Resident Services Director is responsible for assisting with the supervision and coordination of resident health services while serving as a resource and liaison for residents transitioning throughout the continuum of care. The position evaluates resident needs, coordinates care and supportive resources, supports appropriate placement and transitions, and assists with the overall supervision of nursing care in the community.

• Responds to emergency calls as necessary.
• Makes routine rounds to help ensure appropriate care and a safe, clean resident environment.
• Assesses and monitors resident needs and helps ensure residents are receiving appropriate supportive services and are placed at the appropriate level of care within the community.
• Collaborates with residents, families, physicians, nursing staff, and other community departments to identify changing needs, recommend additional services, and coordinate transitions within the continuum of care.
• Coordinates communication with physicians, families, and appropriate staff regarding resident status and changes in condition, including required notifications.
• Coordinates, conducts, and documents resident assessments, care plans, and care conferences in conjunction with or in the absence of the Director of Health Services.
• Coordinates admissions, discharges, and transitions to and from healthcare providers and other levels of care, ensuring a plan is in place for anticipated resident needs.
• Coordinates with the Move-in Coordinator and other appropriate departments to ensure required regulatory documentation and assessments are completed prior to admission or transition.
• Assists residents with understanding relevant health-service resources.
• Maintains awareness of and promotes appropriate internal services to support resident wellness, satisfaction, and retention.
• Develops and maintains information regarding internal and external personal service providers entering the community and ensures required documentation is tracked.
• Coordinates new resident orientation and supports a seamless transition into the community.
• Participates in and/or coordinates community meetings related to resident status, risk, care coordination, transitions, and wellness, including interdisciplinary level-of-care meetings, health center stand-up, care coordination, weekly risk, resident health committees, support groups, and director meetings.
• Assists the Director of Health Services with development and implementation of policies and procedures and participates in infection control, safety, and pharmacy rounds and audits.
• Functions as the Director of Health Services in their absence, as assigned.
• Participates in departmental planning and the annual budget; monitors monthly expenditures, reviews operating statements for accuracy, and explains budget variances.
• Attends assigned in-service training and education sessions and performs other duties and responsibilities assigned by the Executive Director, Administrator, or supervisor.
• Remains available to support broader community needs and communicates effectively with residents, families, staff, and external providers while using sound judgment, maintaining appropriate boundaries, and working effectively under pressure.

These efforts support residents’ wellness needs relating to mind, body, and soul and may also positively affect employees.

• Current RCFE Administrator License
• Current CPR certification.
• Three to five years of experience in long-term care, post-acute care, senior care, or another healthcare setting preferred, with a good understanding of applicable regulations.
• Three to five years of organizational or management experience preferred.
• Staffing, scheduling, and supervisory experience preferred.
• Experience or education in Social Work preferred.
• Ability to problem-solve, make ethical recommendations, define appropriate boundaries, and exercise sound judgment.
• Excellent verbal and written communication and organizational skills.

• Knowledge of resident care practices, care planning, health-services coordination, and applicable senior-care regulations.
• Ability to coordinate across departments and external providers while maintaining resident confidentiality.
• Ability to organize multiple priorities, work effectively with frequent interruptions, and respond appropriately under pressure.

• Must be able to sit, stand, and walk throughout the workday, including standing/walking for extended periods as resident and community needs require.
• Frequent standing, stooping, bending, stretching, reaching, and squatting may be required.
• Must be able to lift, transfer, turn, and position residents weighing up to 50 lbs. unassisted.
• Must be able to lift and carry up to 20 lbs. frequently and up to 50 lbs. occasionally and push or pull up to 50 lbs. unassisted.
• Must be able to perform detailed work with frequent interruptions.
• Employees whose responsibilities require use of KN95 or N95 respirators must meet applicable medical clearance and fit-testing requirements.
