Role Overview
Develop person-centered care plans, including prioritized problem lists, SNAP, client-reported goals, measurable and achievable objectives, and interventions.
Identify and prioritize individual needs and problems.
Responsible for the client's aftercare plan and make decisions based on the best interest of the client.
Communicate with aftercare programs and business development, facilitating aftercare coordination and coordination of care between programs.
Lead and provide group therapy sessions utilizing Evidence Based Practices.
Manage a caseload of up to 12 clients, with minimum of weekly aftercare planning sessions, prepare clinical documentation accurately and within agency time frames.
Coordinate and manage client transfers to higher or lower levels of care and discharges against clinical advice and intervene during crisis intervention situations.
Initiate and develop comprehensive transition and discharge plans and summaries for assigned caseload.
Supervise and participate in the agency’s Quality Management system, Orientation, staff meetings, and in-service training.
Provide accurate assessment and referral when appropriate.
Provide appropriate treatment interventions relative to the specific needs of each client.
Adhere to code of ethics, legal aspects of clinical practice, professional standards, duty to warn, abuse and neglect reporting policies and procedures, and exercise professionalism at all times.
Requirements
A Bachelor’s degree in a behavioral health or related field (Master’s preferred).
Successful completion of the ADP Selective Screening Services background check as well as any other state, federal or company required background/record check.
Possess valid driver’s license with ability to be insured by our auto insurance policy which includes, but not limited to, the following requirements: 4 continuous years of driving experience, 21 years of age or older, less than 3 moving violations in the last 3 years, and no more than 1 major violation in the last 3 years.
Excellent written and verbal communication skills.
Knowledge of clinical and medical terminology related to behavioral health.
Proficiency with Microsoft Office, Adobe, and G Suite applications.
