About this role
Role Overview
Provide case management services to individuals with serious mental illness (SMI)
Conduct regular outreach to assigned members to build rapport and assess needs
Link members to needed services/resources and provide short-term support (up to 6 months)
Respond to complex cases and account specific requests
Conduct assessments to identify individual needs and develop care plans
Monitor and evaluate effectiveness of care plans and modify as needed
Coordinate with internal and external resources to meet identified needs
Requirements
Requires MA/MS in social work counseling or a related behavioral health field or a degree in nursing
Minimum of 3 years of clinical experience in social work counseling
Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist
Previous experience in case management and telephonic and/or in person coaching
Experience with complex psychiatric and substance abuse disorders
Knowledge of behavioral health systems of care in Virginia strongly preferred.
