Care Manager, RN

Role Overview

• Develops, assesses, and facilitates complex care management activities for primarily physical needs members
• Evaluates the needs of the member, barriers to accessing appropriate care
• Develops ongoing care plans/service plans and collaborates with providers
• Provides resource support to members and care managers
• Facilitates care management and collaborates with appropriate providers

Requirements

• Active Florida RN license
• 2–4 years of related experience
• Pediatric experience for Sunshine Health (FL) only
