Role Overview
Assess and evaluate member's needs to achieve and maintain a wellness state.
Guide members/families toward resources and facilitate interaction with them.
Employ a variety of strategies and techniques to manage a member's physical, environmental, and psycho-social health issues.
Resolve barriers that hinder care and oversee patient care through assessments and evaluations.
Create member care plans and follow established guidelines/procedures.
Requirements
Must reside in the state of Kentucky or in a county that is contiguous to the Kentucky state border in Ohio, Indiana, Illinois, Missouri, Tennessee, Virginia, or West Virginia.
Ability to travel to member's residence within 30 to 40 miles in the state of Kentucky.
An active, unrestricted Registered Nurse (RN) license in the state of Kentucky.
1+ years of maternal child nursing experience such as labor and delivery, neonatal ICU, postpartum/nursery.
Intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel.
Knowledge of community health and social service agencies and additional community resources.
Preferred Qualifications: BSN, 3+ years of in-home assessment and care coordination experience, experience with health promotion, coaching and wellness, previous managed care or Medicaid health plan experience, Certification in Case Management (CCM).
