Role Overview
- Supervise and support RNs, LPNs, aides, and therapists
- Oversee patient plans of care and coordinate services with physicians
- Review documentation and OASIS for accuracy and compliance
- Lead case conferences and provide clinical coaching
- Assist with hiring, onboarding, and staff development
- Step into the field when needed to support care
Requirements
- Active RN license (state specific)
- Associate Degree in Nursing (BSN preferred)
- 2+ years Home Health or Hospice RN experience
- Supervisory/preceptor experience preferred
- Reliable transportation and local travel
- Must be able to lift, pull, and push 50-100 lbs
