Conduct regular field visits to engage and build rapport with chronically homeless individuals.
Work with clients to develop goals and service plans.
Provide referrals and direct assistance to support clients applying and/or attending appointments for mental health services, medical care, substance treatment, vital documents, benefits, legal/immigration services, and permanent housing.
Document all interactions with or on behalf of clients, monitor and document client progress toward service plan goals.
Maintain familiarity with social services and resources available both on-site and in the community.
Coordinate client care with other service providers; attend regular case conferences with clients and other service providers, as required.
Attend regular supervision sessions with direct supervisor.
Drive agency vehicle to and from client locations and appointments
Perform additional duties as assigned.
Requirements
Bachelor’s degree in social work or a related field of study
Valid driver’s license which has been held for at least 1 year; in the last 3 years, candidate must have obtained no more than 3 license violation points
One-year of case management or related work experience preferred.
Ability to engage and build rapport with populations with high support needs.
Strong oral and written communication skills
Detail-oriented and resourceful with strong time management and organizational skills
Proficiency in Microsoft Office suite and other standard business technology
Bilingual/multilingual preferred, but not required.
Valid driver’s license is required for this position.
