Establishes and maintains strong, trusted relationships with physicians, advanced practice providers, hospitals, skilled nursing facilities, assisted living communities, home health agencies, and other post-acute partners.
Serves as a primary point of contact for community partners, ensuring consistent communication, responsiveness, and follow-through.
Educates referral sources on Meridian’s clinical services and care models.
Identifies opportunities to expand service lines, improve care coordination, and strengthen alignment with referral partners.
Supports timely and appropriate referrals by facilitating communication between referral sources, operations, and clinical teams.
Works collaboratively with internal teams to ensure smooth onboarding of new facilities, providers, and programs.
Assists with resolving barriers to referral flow, operational readiness, and service utilization.
Conducts ongoing market assessments to identify trends, competitive dynamics, and growth opportunities within the assigned territory.
Provides feedback and insights to leadership regarding community needs, service gaps, and partnership opportunities.
Represents Meridian professionally and ethically at community events, meetings, and industry forums.
Builds and protects Meridian’s reputation for clinical excellence, transparency, and integrity.
Maintains strict confidentiality of patient, provider, and organizational information.
Participates in internal meetings, trainings, and cross-functional initiatives as required.
Maintains regular, reliable attendance and flexibility to meet market and organizational needs.
Requirements
Bachelor’s degree in healthcare, business, marketing, or a related field preferred (relevant experience may substitute).
Minimum of 1–3 years of healthcare relationship management, market development, or provider-facing experience.
Experience in post-acute care, value-based care, physician services, hospice, palliative care, or care coordination strongly preferred.
Strong interpersonal, communication, and presentation skills.
Ability to work independently while collaborating effectively with cross-functional teams.
Highly organized, self-directed, and comfortable operating in a fast-growing healthcare environment.
Proficient with CRM systems, Microsoft Office, and healthcare communication tools.
Ability to function effectively in an interdisciplinary, physician-led care model.
