Deputy Director – Care Coordination

Role Overview

• Responsible for assisting with the overall administration of the CHN Care Coordination program.
• Clinical supervision of the Care Coordinators and Interns.
• Coordination of client services.
• Staff recruitment and hiring.
• Staff training.
• Community and case finding.
• Weekly, monthly and quarterly reporting of program information to the Program Director.
• Implementation of the program policies and procedures.

Requirements

• Masters degree in Social Work preferred or Masters level with commensurate clinical training and experience.
