Ensure the seamless delivery of services and support to clients
Utilize intervention and communication skills to engage with clients and families
Document client activities in the Electronic Health Record (Best Notes) within 24 hours
Collaborate with clinical teams to develop the transition and discharge plans
Arrange follow-up appointments and referrals to support clients’ continued care
Provide education and emotional support to clients and their families
Organize and maintain company databases
Generate reports to support compliance efforts
Ensure adherence to licensing and accreditation requirements
Implement and uphold office administrative systems
Support the administrative staff
Requirements
Bachelor’s degree in Human Services or related field
Ability to pass a criminal background check and other background checks required by AMFM Healthcare
Valid driver’s license, 4+ years of driving experience, fewer than 3 moving violations in the last 3 years, and no more than 1 major violation in that time
CPR/First Aid Certification and TB clearance required
Strong communication skills—both written and verbal
Professional, compassionate, and organized with a knack for multitasking
Proficient in Microsoft Office, Google Workspace, and Adobe
Familiarity with clinical/medical terminology a plus
