Support and care for clients with empathy and expertise to improve their personal health and wellbeing goals.
Undertake coordination of supports and wellbeing services for a highly engaged and motivated client population.
Provide initial early intervention Triage with clients, supporting both physical and mental health services.
Undertake relevant case coordination activity including liaison with all stakeholders; provision of email updates; and creation of appropriate documentation such as treatment plans or other documentation as required.
Requirements
Experienced Registered Nurse with full registration
Hold a current and valid Working With Children Check (WWCC)
Evidence of completion of First Aid (level 2) or in circumstances where staff have obtained “Basic Life Support” training within a hospital setting
Caring and empathic nature.
Experience care coordination or occupational rehabilitation desirable.
Well-developed written and verbal communication skills.
Good IT and time management skills.
Strong customer service focus.
