Manage inbound referrals from health system and provider partners, ensuring accurate intake and documentation
Engage referred patients and assist with scheduling their Midi appointments
Ensure referrals move through established workflows efficiently and without delays
Serve as a point of contact for patients navigating care between Midi and its health system and provider partners
Answer patient questions about Midi services, referral pathways, and next steps in care
Provide clear, supportive communication to ensure a positive patient experience
Manage the dedicated Zendesk inbox and voicemail for inbound referral communications
Share updates and operational feedback with the Commercial team regarding referral activity and workflow improvements

2–4 years of experience in care coordination, referral management, patient navigation, or patient support
Experience working in a medical practice, healthcare organization, or digital health environment
Daily hands-on experience using Zendesk or a similar ticketing platform to manage patient or customer support queues
Strong experience communicating with patients via phone and written channels
Comfortable managing high volumes of inbound requests and patient communications while maintaining a positive patient experience
Ability to follow structured workflows and SOPs while identifying opportunities to improve operational processes
Experience working with EHR systems and operational tools
Strong communication and cross-functional collaboration skills
Comfort working in a fast-paced, growing healthcare organization.
