RN Care Manager

Role Overview

• provides assessments, planning, interventions and follow-up for patients and family members
• addresses potential or actual clinical and/or psychosocial needs
• facilitates adherence to a treatment plan to optimize patient health
• collaborates with the interdisciplinary team to ensure safe and seamless transitions of care
• documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements
• maintains current knowledge of pediatric specialty practice, healthcare economics, trends, and reimbursement methodologies

Requirements

• Bachelor in Nursing (BSN)
• 3 years nursing experience
• Current California RN license
• Case Management Certification (CMC, ACM or ANCC) required within 12 months of hire or eligibility
• Master's in Nursing, or other Healthcare Related field preferred
• 1 year Care Management in Pediatrics preferred
• 5 years clinical experience in one or more of the facility's service lines (SCL, Pediatric Orthopedic Burn Care, cleft lip and palate) preferred
