About this role
Role Overview
Build strong, supportive relationships with assigned CBOs throughout the SCN readiness process
Use standardized SCN onboarding approaches while adapting to CBO-specific needs
Support CBOs in completing readiness assessments and developing Dynamic Needs Maps identifying pre-onboarding action items
Guide CBOs in completing Dynamic Needs Map items, including project planning, workflow development, and resource creation
Coordinate technical assistance to strengthen organizational capacity for screening, navigation, and service delivery within the SCN
Assist CBOs with accessing the correct technical assistance sessions and materials
Monitor progress of onboarding CBOs using dashboards, reports, and milestone tracking tools
Provide clear, consistent updates on CBO progress and needs to the SCN Project Director and FLIPA leadership
Support data collection, interpretation, and communication of key findings across the project lifecycle
Participate in peer learning and group-based improvement strategies with fellow Partner Liaison Specialists and SCN team members
Identify trends, challenges, and opportunities for process improvement across the CBO network
Support the development and execution of learning community strategies to facilitate shared knowledge, standardization, and ongoing improvement
Work effectively across diverse organizations, geographies, and organizational cultures
Support leadership in developing enhanced or tailored technical assistance based on emerging CBO needs
Complete other duties as assigned.
Requirements
Associate or bachelor’s degree in a relevant field (business, public health, healthcare administration) or equivalent experience
Minimum of 3 years of experience in relationship management, project management, or a related field
Experience within the New York State healthcare system (population health, behavioral health, FQHCs, etc.) is required
Familiarity with regional CBOs, local healthcare systems, and partner agencies preferred
Prior experience with large-scale implementation grants or projects preferred
Strong organizational, leadership, and project management abilities
Ability to manage multiple priorities with minimal supervision
Outstanding communication and interpersonal skills
Demonstrated ability to build relationships and support organizational change
Capacity to gather and communicate project information clearly to diverse audiences
Experience with full project lifecycle management
Proficiency in Microsoft Office and other business software applications
Self-motivated, resourceful, and effective in a remote work environment
Commitment to equity, social justice, and making meaningful impact in underserved communities.
Benefits
Role requires approximately 50% travel for site visits that support community engagement and relationship building
Periodic travel for meetings across the Finger Lakes Region and New York State
