Provide in-home physical therapy evaluations and treatments
Assess patients’ strength, mobility, balance, and ability to function safely at home
Create and update individualized plans of care that support independence and recovery
Help patients regain mobility and confidence in their home environment
Educate patients and caregivers on exercises, safety, and fall prevention
Document visits and evaluations in WellSky/Kinnser in a timely manner
Collaborate with nurses, therapists, and the broader care team to support patient outcomes
Supervise Physical Therapy Assistants (PTAs) and support their clinical development
Participate in care planning, case conferences, and discharge planning
Requirements
Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or DPT)
Active California Physical Therapist license
CPR/BLS certification
Strong communication and organizational skills
Reliable transportation and willingness to travel to patients’ homes
Comfortable working independently in a home health setting
Home health experience preferred, but not required
