About this role
Role Overview
Provide in-home occupational therapy evaluations and treatments according to the physician’s plan of care
Assess patients’ strength, coordination, mobility, and ability to function safely at home
Develop and update individualized treatment plans based on patient goals and progress
Help patients improve independence with daily activities such as dressing, bathing, mobility, and home safety
Educate patients and caregivers on exercises, adaptive techniques, and safe movement
Document evaluations, visits, and progress in WellSky/Kinnser in a timely manner
Communicate patient progress, concerns, and changes in condition to the care team and physician
Supervise COTAs in accordance with state guidelines
Participate in care planning, case conferences, and discharge planning
Requirements
Graduate of an accredited Occupational Therapy program (Master’s or Doctorate in Occupational Therapy)
Active California Occupational Therapist license
Current CPR/BLS certification
Reliable transportation and willingness to travel to patient homes
Home Healthcare experience
Experience documenting in the Kinnser/Wellsky EMR preferred, but not required
