Role Overview
- Serve TennCare members with Intellectual and Developmental Disabilities enrolled in a 1915(c) Waiver program.
- Coordinate care across multiple partners and benefit programs.
- Manage complex situations where additional support is needed.
- Oversee challenging cases to ensure effective interventions and appropriate service utilization.
Requirements
- 3+ years of experience in a clinical setting.
- An active RN license (TN or compact states).
- Ability to work independently from home with face-to-face visits based on members acuity.
- 5 years experience in the health care industry.
- 2 years experience in IDD required for Select Community.
