About this role
Role Overview
- Engage with members to remove barriers to Social Determinants of Health (SDOH)
- Work with members to establish goals for addressing SDOH
- Support members in selecting appropriate provider/programs
- Act as liaison between referral sources and health plan
Requirements
- BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field
- Minimum of 2 years experience with marketing, job development, job placement or social work
- Knowledge of Voc Rehab preferred
- Previous Behavioral Health experience preferred
- Previous experience working with SDOH preferred
- Proficiency in using electronic medical records systems (EPIC)
- Proficient in MS Teams, Excel and SmartSheet
