Conduct in‑clinic, face‑to‑face visits with dialysis patients and family members to understand their daily challenges and support needs
Provide education in a compassionate, conversational manner that encourages openness to in‑home care support
Establish and maintain strong relationships with customers by delivering exceptional service and understanding their unique needs through care coordination
Meet or exceed monthly admission targets and key performance indicators (KPIs).
Guide customers through the referral process, including identifying needs, providing service recommendations, and achieving admissions
Demonstrate continued engagement with clients and families, assessing satisfaction, and identifying/resolving services issues to ensure retention of client with Personal Care Services
Accurately enter and update customer information and engagement activities in the company’s Customer Relationship Management (CRM) system (BOLT)
Identify opportunities to cross-sell or upsell additional services based on customer needs; coordination through Home Care Home Base and connecting with the care team.
Participate in ongoing training sessions to stay updated on product knowledge, sales techniques, and industry trends
Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
Requirements
Graduate of an accredited LVN/LPN Program
Current state license as LPN/LVN
Minimum of one year experience in home care or health care industry preferably a nephrology nurse; discharge planning experience and/or healthcare sales experience
Previous experience in home care, care coordination, case management, or patient education
Familiarity with stages of Chronic Kidney Disease (CKD) and dialysis workflows
Sales, outreach, or client‑engagement experience (healthcare preferred)
In-depth knowledge of Medicare/Medicaid (e.g. MCO, MLTC, other payer types, etc.), home care benefits, rules and regulations across multiple states
Ability to work with various computer and data entry systems, including Microsoft Office (Word, Excel, PowerPoint), computer systems and other database software
Strong communication skills; both written and verbal
Bilingual preferred
Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with the laws of the State.
Sit, stand, bend, lift and move intermittently and be able to lift at least 25 lbs.
90% travel to local service locations (assigned to a regional area)
