achieve and maintain wellness state by guiding members/families toward and facilitating interaction with resources appropriate for the care and wellbeing of members
employ a variety of strategies and techniques to manage a member's physical, environmental and psycho-social health issues
resolve barriers that hinder care
ensure patient is progressing towards desired outcomes by monitoring patient care through assessments and evaluations
create member care plans
support the Transition of Care (TOC) program to ensure continuity of care for members as they move between care settings
assist with HEDIS completion follow up visits after TOC and conduct field visits based on member needs and program requirements
Requirements
Bachelor's degree
3 or more years of experience working as a licensed Social Worker
Hold an active and unrestricted license as a Behavioral Health Professional in South Carolina, such as; Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Social Worker (LSW)
Master's degree in a behavioral-health field, such as social work, psychology, or related health discipline from an accredited university (preferred)
1 year of managed care experience (preferred)
Transition of care (TOC) experience (preferred)
CCM certification (preferred)
Experience working with the geriatric population (preferred)
Knowledge of community health and social service agencies and additional community resources (preferred)
