Physical Therapist – Home Health

Role Overview

• Plan and administer prescribed physical therapy treatment for patients in their homes
• Assess and document patients' physical strengths to support physician evaluation
• Develop and implement rehabilitation programs
• Document patient observations and report progress to the care team

Requirements

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Current and unrestricted Physical Therapy license
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
